It is important to know where to begin and how. That is where Eiros Group can help you begin getting started with Adult Disability Services.
Step 1: Apply for Medicaid
Through NJ Family Care and the Division of Medical Assistance and Health Services (New Jersey Department of Human Services (DHS), begin the application process to receive Medicaid Benefits for Adults with Disabilities.
Step 2: Check Qualifications for DDD Services
Also covered through the New Jersey Division of Developmental Disabilties, this comprehensive Question & Answer page will help you navigate through DDD Guidelines, Qualifications and the Application process.
Step 3: Apply for DDD Services
It is important to note there is either a FULL Application or a SHORT Application. BOTH formats of the application must be printed on paper and completed. Once the application is completed and signed, it may be emailed along with all required supporting documentation (scanned and attached to an email), or sent by regular mail (with copies of all documentation) to a Community Services Office (click here to view the CSO for your area).
